Your pre-operative assessment

As part of your pre-operative assessment, you might need to complete an online health questionnaire. This will help you prepare for surgery and minimise any unnecessary travel to the hospital.

If we have your mobile number, we will send you a secure text message from Circle with a link to the online health questionnaire for you to fill out. Alternatively, you will receive an email from Ultramed, our partner in managing online health questionnaires. You can rest assured that the link is secure, and only the healthcare professionals directly involved in your care will be able to access your information.

We kindly encourage you to complete the health questionnaire as soon as you receive the link. After completing the questionnaire, you will be contacted promptly by a member of our team and advised on the next steps of your journey. If you experience any difficulties or are unable to complete the health questionnaire using the link, please don’t hesitate to contact us on 01753 336457. Our pre-operative assessment team is here to support you.

Community fundraising

Each year we support and contribute to the fundraising of vital hospices and charities in Windsor, Berkshire and surrounding areas, including Alexander Divine and The Windsor Homeless Project. Please reach out to a member of staff if you would like to find out more about our upcoming charity work.
